After filling out (editing) the Evaluation Form, you are to upload ONLY (1) document. Please DO NOT upload multiple files. If you are in need of combining PDF files, there are many free websites that allow you to do this. We know that no one is perfect, therefore, be honest and open with your evaluation. Your evaluation form should NOT have all 5’s or 1’s (correspondingly).
