I was given a template worksheet that has to be filled in based off a diagnosis

I was given a template worksheet that has to be filled in based off a diagnosis of                  peritonsillar abcess. The worksheet needs to be filled out as if a female patient with severe throat pain comes to the emergency department and is weak and unable to eat. It is not a paper, just a template that needs to be filled out baed on this patient and her diagnosis.
