CC:  “My periods are crazy, and i havent been feeling myself lately I thin k is the menopause” 
Patient is a 45 y/o female,  who came complaining irregular menstrual cycles for the past 6 months more or less, She reports that her periods have become unpredictable, sometimes even skipping a month. She also experiences hot flashes several times a day, which are worse at night which affects her sleeping she also states notice some mood changes including irritability, along with a decrease in her energy levels and occasional difficulty concentrating at work. She denies any significant weight changes, but she has experienced some bloating and breast tenderness.  We ordered full hormonal panel, we will start on low dose of oral contraceptives and will consider hormone therapy in the future if needed after test results. 
Diagnosis:  (perimenopause)
 N95.9 – UNSPECIFIED MENOPAUSAL AND PERIMENOPAUSAL DISORDER
