A Focus SOAP Note is due on a patient you have seen in the clinic with your preceptor. A paper copy of the SOAP Note Template is provided in the Clinical Documents and Forms tab in the course. All information pertinent to the focused visit should be included in your SOAP Note. The patient used in the Focus SOAP Note should be documented on the clinical log for the week as well.
