This is a comprehensive annotated SOAP note based on an actual patient (all PII removed). I have included medical record notes for the patient, assignment instructions, and an example of a previous SOAP note. PLEASE note that the example is just a reference to have an idea of the expectation, however this SOAP note outline is slightly different from the example and needs to be followed per the instructions for this particular assignment. If information is missing, you can make it up within reason and with what fits the narrative for this patient.
